Since each person's health care needs are unique, CHCS has developed a large variety of available services designed to meet your individual needs.

Nursing Care
All procedures that require the skills of a registered or licensed nurse: follow-up care after hospitalizaton, dressing changes, diabetic teaching, catheter care, intravenous therapy, exercise programs and health teaching are some of the services provided.

Paraprofessional Health Care
Certified Home Health Aides (supervised by a professional nurse) will prepare special diets, bathe the patient, and help skillfully with the care of the bed-ridden or chair-bound patient.

Homemaker Services
A mature, experienced staff person may do light housekeeping, budgeting, shopping, and, in some circumstances, assisting with child care.

Rehabilitation Services
Physical, Speech, and Occupational therapy helps the individual needing rehabilitation services as part of their treatment plan. We are committed to providing these services in all areas; however, services are based on staffing resources.

Medical Social Services
Social workers assist patients and families with the psycho/ social aspects of an illness, as well as coordination and referral to other helping services.

Other Assistance
Helpful assistance is provided in obtaining:
  • medical equipment and supplies
  • help with insurance
  • pre-discharge in-home assessment
  • hospital discharge planning with family and hospital staff
  • medication review/teaching
  • case management
  • coordinating other community resources to meet the total needs of the client.
