Your doctor may recommend a plan of health care at home. You, a member of your family, or even a friend, may request services. In every case, a member of the home health staff will make an initial visit to the home to evaluate your specific needs. If home health care will benefit you, a plan of care is developed based on medical orders from your doctor.

For example, one such plan might require a weekly visit by a registered nurse and help from a home health aide every day or two.

Another plan might specify one or more visits from a therapist and follow-up visits by a home health aide to help with exercises. Progress reports are reviewed regularly by your doctor and services continue until they are no longer needed.

  • Psychiatric Nurses
  • Medical Social Workers
  • Certified Home Health Aides
  • Physical Therapists
  • Homemakers
  • Speech and Occupational therapy services are also available based on staffing resources.
