Helping you transition from hospital to home comfortably. CHCS home health professionals will help you recover from hospitalization as you regain your strength and energy. They can also make recommendations to help you remain safe in your home. Hospitalizations occur for many reasons or conditions.
CHCS uses customized treatment plans to meet your personal medical needs when returning home. Specific conditions may include heart related diseases or surgeries, respiratory related diseases, circulatory problems, infections, orthopedic conditions, wounds, and generalized weakness.
Treatment focuses on your comfort and safety as you recover at home from your hospitalization.
CHCS provides professional assessment and education for you and your caregiver related to many conditions.
Professionals can provide specific assessment and education on conditions such as:
- Heart Failure
- Heart Attack
- Chest Pain
- Chronic Lung Conditions
- Circulatory Conditions
We can help you better understand your medications when you return home from the hospital and can work with your doctor to help ensure you are taking the right medications safely.
Post Joint Replacement Rehabilitation
Either directly from hospital or skilled nursing facilities, the therapy team at CHCS will work closely with your surgeon to get you quickly back on your feet, doing the activities you enjoy most. CHCS provides comprehensive physical therapy, occupational therapy, nursing, and home health aide services as you recover from joint replacement surgeries, including, but not limited to, hips, knees, and shoulders. Therapists also work with individuals living with arthritis and osteoporosis through positioning, pain relief techniques (from ultrasound to manual therapies), and specifically designed exercise programs.
Your team of physical therapists, occupational therapists, and nurses will customize a treatment program to meet your needs.

- Teach stretching and strengthening exercises
- Assess and teach how to move about your home safely

CHCS nursing professionals provide you with support and assistance with short and long-term antibiotic, hydration, and/or nutrition therapy. Nurses also provide individual assessment and education for patients requiring feeding tubes. They will work with families and physicians to support these treatments.
Telehealth services provide an extra measure of monitoring on days that the nurse does not visit.
Telehealth is a service that CHCS uses to improve patients' health care by monitoring their conditions on days that a nurse is not visiting. There are many benefits to Telehealth services. Telehealth allows CHCS to identify early signs of trouble, institute treatment, and prevent unnecessary hospitalizations. It allows CHCS communication to keep physicians updated on their patients' conditions.
Interactive Telehealth sessions make patients informed consumers, increasing their confidence and ultimately, improving their independence and quality of life.
CHCS provides wound care based on best practices for surgical and non-surgical wounds.
CHCS provides state-of-the-art wound care management with consultation of a nurse practitioner, who is a certified wound specialist. CHCS uses enhanced digital photography to assess and advance treatment of the wound to enhance patient comfort and reduce healing time.
Wounds may be surgical or non-surgical. Non-surgical wounds may include bed sores, skin ulcers, or traumatic injuries. CHCS nurses work with the wound specialist and physicians to provide personalized wound care.
Community Health and Counseling Services will provide community health services that are needed and valued by the communities and individuals we serve.
