Job Description
The ACT Community Support Worker (CSW) provides supportive counseling, case management, outreach, skill teaching, family support, coordination/advocacy for services, education, and crisis support to persons with severe and prolonged mental illness. The goal of these services is to assist individuals in obtaining the support, skills, and services necessary to improve their level of functioning within their communities and to decrease the need for hospitalization and/or incarceration. The services are delivered in a variety of home, office, and community based settings, depending on the needs of the individual.
