CCBHC Referral Specialist

Job Description

Will be responsible for managing referrals that come to our CCBHC services across the service locations. This role is responsible for managing internal and external referrals, conducting basic screenings to determine appropriate services, and collecting initial data necessary for processing referrals. This role involves scheduling individuals with providers and ensuring effective communication between internal and external providers to meet client needs. This role works collaboratively with interdisciplinary care teams to ensure clients receive the necessary services and support to improve their health outcomes and quality of life. A high school diploma or GED is required, secondary education in human services a plus.

Hourly Salary – $20.00
