Hospice Interdisciplinary Team
The Hospice Interdisciplinary Team is the guiding hand to ensure you receive what you need at the end-of-life.
The Interdisciplinary Team (IDT) includes doctors, nurses, hospice aides, social workers, spiritual advisors, therapists, and trained volunteers. Hospice services are provided as needed and as approved by the IDT. Services are available 24/7/365 to provide support and care to patients and their families.
The IDT works together and focuses on the neeeds of dying patients and their loved ones needs -- physical, emotional, or spiritual. The goal is to help keep patients as pain-free as possible until death. The hospice team develops a care plan that meets each patient's individual needs for pain management and symptom control.

Community Health and Counseling Services will provide community health services that are needed and valued by the communities and individuals we serve.
